Key Responsibilities Conduct comprehensive assessments of clients' needs, strengths, and barriers to recovery. Develop individualized, client-centered recovery plans in collaboration with clients. Connect clients to appropriate community resources, including treatment programs, housing, employment services, and support groups. Provide ongoing case management, monitoring client progress and adjusting plans as needed. Offer supportive counseling and crisis intervention as required. Educate clients and their families about addiction, recovery processes, and available resources. Maintain accurate and confidential client records in accordance with agency policies and regulations. Collaborate with treatment providers, healthcare professionals, and other stakeholders to ensure integrated care. Advocate for clients' needs within the healthcare and social service systems. Facilitate client engagement in evidence-based recovery practices and life skills development. Requirements Bachelor's degree in Social Work, Psychology, Counseling, or a related human services field. Minimum of 3 years of experience in case management, preferably in addiction recovery or mental health services. Knowledge of addiction, recovery principles, and evidence-based treatment modalities. Familiarity with community resources and referral networks in the **Las Vegas, Nevada, US** area. Strong assessment, planning, intervention, and evaluation skills. Excellent interpersonal, communication, and active listening abilities. Proficiency in using electronic health record (EHR) systems and other relevant software.
