Work Environment
As a Licensed Independent Social Worker, you will work remotely from the comfort of your home while being part of a dedicated and collaborative team. Key Responsibilities
In your role, you will:
• Conduct comprehensive psychosocial assessments to evaluate patients' mental health status, strengths, and needs.
• Utilize evidence-based therapies, including cognitive-behavioral therapy and dialectical behavior therapy, to provide essential therapeutic interventions.
• Collaborate with a diverse team of psychiatrists, nurses, and other healthcare professionals to ensure a cohesive approach to patient care.
• Guide patients and their families in understanding diagnoses, treatment options, and medication management.
• Deliver individual, group, and family therapy sessions, employing a range of evidence-based modalities tailored to each patient's needs.
• Design and implement treatment plans in partnership with the treatment team, prioritizing patient goals and preferences.
• Aid patients in navigating the complexities of the mental health system, advocating for their rights and essential needs.
• Facilitate therapeutic group sessions, psychoeducation workshops, and discharge planning meetings.
Patient Population / Client Focus
You will primarily support patients facing end-of-life challenges and their families, ensuring they receive the emotional and mental health support they need. This role requires a deep understanding of grief, loss, and the psychological aspects associated with terminal illnesses, along with the ability to provide compassionate care and effective coping strategies.
Qualifications
To be considered for this role, you will need:
• A Master's degree in Social Work from an accredited institution.
• An active state licensure as a Licensed Independent Social Worker (LISW).
• A comprehensive knowledge of mental health disorders, treatment methods, and crisis intervention techniques.
• The ability to collaborate effectively within a multidisciplinary team while demonstrating strong interpersonal skills.
Preferred Experience
While not mandatory, candidates with prior experience in hospice, palliative care, or end-of-life counseling are highly preferred. Familiarity with remote therapy platforms and telehealth practices will also be beneficial, enabling you to provide effective and timely support to patients.
Compensation and Benefits
