Position Overview – Licensed Practical Nurse (LPN), Home Health
The Home Health Licensed Practical Nurse (LPN) is the frontline caregiver delivering skilled nursing services directly in patients’ homes. You will collaborate closely with Registered Nurse (RN) Case Managers, Home Health Aides (HHAs), physicians, and multidisciplinary team members to assess, plan, implement, and evaluate individualized care plans.
