This position offers the opportunity to: • Work with a dedicated healthcare team • Gain valuable experience in healthcare administration • Directly impact the well-being of those we serve • Develop skills in care coordination and reporting • Contribute to improving community health outcomes As a Facilitator, you'll be responsible for managing day-to-day tasks, tracking client activities, and submitting reports to the Department of Mental Health. You'll serve as a key resource for the Healthcare Homes team and other clinical teams, ensuring smooth operations and compliance with regulations. This role requires a compassionate individual who can balance administrative duties with the mission of providing quality care to those in need. Responsibilities • Coordinate day-to-day tasks of the Health Home program, ensuring efficient operations • Track client activities and prepare monthly reports for the Department of Mental Health • Provide clerical support and assistance to the Healthcare Homes Director and team • Maintain user accounts and monitor agency usage as a Cyber Access Practice Administrator • Offer technical support for Care Manager and Cyber Access systems • Ensure timely completion and submission of required client screenings and reporting measures • Attend state-required meetings and trainings to stay current with program requirements • Collaborate with the Quality Assurance team to maintain compliance with established procedures • Assist in distributing reports to treatment teams and community providers as needed Education and Experience • Education: High school diploma or GED required Additional Qualifications • Proficiency in Microsoft Office and Microsoft Access • Excellent communication and organizational skills • Ability to implement innovative programs and adapt to changing requirements • Must complete New Hire Orientation and all required training, including Relias, at the beginning of employment and annually thereafter • Valid driver's license, acceptable driving record, and current auto insurance required • Must successfully pass a background check, including criminal record and abuse/neglect screening • Physical ability to sit and stand for extended periods (2-4 hours), occasionally lift up to 50 pounds, and perform other essential job functions Position Perks & Benefits • All-Inclusive Employee Benefits Package - A robust full-time employee benefits package encompassing health, dental, vision, retirement, disability, life insurance, wellness program, and more • Telemedicine – 24/7 phone, web, or mobile app medical, behavioral health, & dermatology visits • Employee Assistance Program – 24/7 counseling services, legal assistance, & financial consultation for you and your household at no cost • Workplace Culture - An environment cultivating employee wellbeing, valuing each individual's humanity, and actively promoting healthy, joyful workforce engagement We are an Equal Employment Opportunity Employer.
