Position Overview
The Home Health Physical Therapist (Full‑Time) will be the frontline champion of rehabilitative care for patients recovering from injuries, surgeries, chronic illnesses, or age‑related functional decline. Key Responsibilities
- Patient Assessment & Evaluation: Review medical histories, conduct thorough physical examinations, and use standardized outcome measures to determine baseline function and set realistic, measurable goals.
- Treatment Planning: Develop individualized, physician‑approved therapy plans that integrate therapeutic exercises, manual techniques, functional training, and education for patients and caregivers.

- Interdisciplinary Collaboration: Communicate regularly with physicians, nurses, occupational therapists, speech‑language pathologists, and care managers to coordinate comprehensive care.
- Supervision & Mentorship: Provide clear written instructions and ongoing mentorship to Physical Therapy Assistants (PTAs) and Home Health Aides, ensuring adherence to the plan of care.
- Documentation Excellence: Accurately record assessments, treatment notes, progress updates, and discharge summaries in compliance with Medicare, state regulations, and CenterWell policies.
- Quality Assurance: Monitor compliance with clinical practice guidelines, participate in peer‑review sessions, and contribute to continuous‑quality improvement initiatives.
- Patient & Family Education: Teach safe home exercise programs, equipment usage, and strategies for fall prevention, empowering families to support long‑term recovery.
- Outcome Reporting: Generate timely progress reports for physicians, clinical managers, and insurance payors, highlighting achievements and any needed plan modifications.
Essential Qualifications
- Graduated from an APTA‑accredited Physical Therapy program with a Doctor of Physical Therapy (DPT) or Master’s degree.
- Current, unrestricted Physical Therapy license in the state of Michigan (or eligibility for licensure).
- Minimum of one year of clinical experience in acute, outpatient, or home health settings; experience in home health preferred.
- Current CPR/BLS certification.
- Valid driver’s license, reliable personal automobile, and appropriate auto‑insurance coverage for travel to patient homes.
- Strong organizational skills, with the ability to manage multiple caseloads, prioritize tasks, and meet documentation deadlines.
- Excellent verbal and written communication skills, including the ability to convey complex clinical information to patients, families, and interdisciplinary team members.
Preferred Qualifications & Additional Skills
- Experience with electronic health record (EHR) systems and tele‑health platforms.
- Specialized certifications such as Orthopedic Clinical Specialist (OCS), Geriatric Clinical Specialist (GCS), or Neurologic Clinical Specialist (NCS).
- Knowledge of evidence‑based home health documentation standards (e.g., OASIS, CMS).
- Proficiency in using assistive devices, adaptive equipment, and home modifications to enhance patient independence.
- Demonstrated ability to work autonomously in ambiguous situations while maintaining high clinical standards.
- Fluency in a second language (Spanish, Arabic, etc.) to serve diverse patient populations.
