$77,000 to $90,000+ with Generous Benefits
$15,000 sign on bonus for candidate with NY State LMSW or LCSW
Remote job after initial hybrid 4 to 6 months training (onsite 2-3 days per week during training)
Training Location: Manhattan, NY
Full-time, Permanent Role - Monday through Friday, Business Hours
Main goal for Floating Care Manager is to address any urgent issues experienced by members during absence of their assigned Care Manager.
Summary:

• Provide care management coverage to Health Plan members whose primary Care Manager is out of the office.
• Performs telephonic assessments of members' medical, psychosocial, physical, and spiritual needs and puts in appropriate interventions.
• Develop, implement, and monitor Person Centered Service Plan, and ensure continuity of care across all healthcare settings by collaborating with the interdisciplinary care team, PCP, member/member's representatives, and specialists.
• Utilize benefit structure and community-based resources to meet member's needs.
• Educate members on disease processes, preventative health, and quality of life interventions.
• Knowledgeable in and adhere to CMS/DOH regulations and policies.

Requirements:

• 3-5 years clinical social experience in long term care, managed care, home care, geriatrics or hospice
• Prior experience in an interdisciplinary service delivery environment preferred.
• Working knowledge of Medicare and Medicaid regulations
• Prior experience in an interdisciplinary service delivery environment.

Salary Range is $77,000 to $90,000 + Gernerous benefits
