About Our Remote Patient Monitoring Program
Our RPM program is designed to provide patients with the tools and support they need to manage their health from home. We equip our patients with devices that allow them to track their daily vitals and respond to symptom and risk screening questions. As an LPN in this program, you'll be responsible for reviewing patient data, identifying potential health concerns, and providing education and support to patients and their caregivers. This is a unique opportunity to apply your nursing skills in a new and innovative way, with a focus on care management and patient education.
Key Responsibilities
- Review patient data daily, including vital signs and survey responses, to identify potential health concerns and escalate issues to RNs or providers as needed.
- Communicate with patients and their caregivers via phone or video calls to provide education, support, and reinforcement of healthy behaviors.
- Document patient interactions and vital signs in our electronic health record system.
- Collaborate with our multidisciplinary care team, including RNs, providers, and other healthcare professionals.
- Participate in program referral processing and daily patient metrics review.
- Deliver high-quality, patient-centered care that meets regulatory requirements and ensures reimbursement.
Requirements and Qualifications
Education and Experience
- Graduate of an accredited school of nursing or education equivalency for licensing.
- At least one year of experience as a Licensed Practical Nurse.
- Remote experience preferred but not required.
- Experience in triage or virtual health is highly desirable.
Licensure and Certification
- Current licensure as a Licensed Practical Nurse (LPN) in Illinois, Missouri, Oklahoma, or Wisconsin.
- Nurse licensure issued by a compact state is also acceptable.
Skills and Competencies
- Excellent communication and interpersonal skills.
- Strong critical thinking and problem-solving abilities.
- Proficiency in electronic health record systems.
- Comfort with technology, including video conferencing and phone systems.
- Ability to work independently in a remote setting.
