Job Summary:

Key Responsibilities:
- Engage with patients to understand their care needs and address healthcare concerns
- Collaborate with medical practice staff and Case Management teams to deliver comprehensive care
- Accurately document patients' medical history and issues in electronic health records (EHRs)
- Meet assigned Key Performance Indicators to drive patient outcome improvements
- Utilize various communication methods to effectively engage high-risk patients
Requirements:
- Certified or Registered Medical Assistant or certified EMT
- Proficiency in medical terminology and primary care physician practices
- Experience with electronic health records (EHRs) and care management analytics
- Exceptional written, visual, and verbal communication skills
- Ability to work independently as part of a patient's care team under supervision
Preferred Qualifications:
- Familiarity with EHR systems such as EPIC, Athena, or Cerner
- Experience in motivational interviewing and behavioral change management
- Strong critical listening skills and empathy in patient coordination
- Ability to manage multiple tasks while maintaining attention to detail
- Proven success in a remote work environment
