About the position
This role involves providing community-based physical therapy services, focusing on helping patients regain their independence and improve their quality of life. Physical Therapists will evaluate patients in their homes, develop individualized treatment plans, and collaborate with a healthcare team to achieve functional outcomes. The position offers the flexibility to manage your own schedule and see the direct impact of your treatment in patients' everyday lives.
Responsibilities
• Evaluate patients in their homes and create individualized treatment plans.
• Improve mobility, strength, balance, endurance, and functional independence.
• Educate patients and caregivers to support long-term recovery.
• Collaborate with nurses, physicians, occupational therapists, speech therapists, and other members of the interdisciplinary team.
• Monitor progress and adjust treatment plans to maximize outcomes.
• Complete accurate and timely documentation.
Requirements
• Active Minnesota Physical Therapist (PT) license.
• Excellent clinical reasoning and communication skills.
• Passion for helping patients achieve meaningful, lasting outcomes.
• Ability to work independently in a field-based setting.
• Valid driver's license and reliable transportation.
