We are growing and seeking experienced Home Health Registered Nurses (RN) to join our PRN per-visit team. This position offers flexible scheduling, competitive per-visit pay, and strong clinical support, allowing you to focus on delivering excellent patient care. Earn Competitive Per-Visit Pay Based on Visit Type We offer a tiered per-visit pay structure, allowing experienced home health RNs to earn more for higher-complexity visits: • Routine Skilled Visits: Starting at $45+ per visit • Higher Pay for OASIS & Complex Visits: • Start of Care (SOC- OASIS): Higher rate • Recertification (OASIS): Higher rate • Resumption of Care (ROC): Higher rate • Discharge Visits: Higher rate Mileage Reimbursement Provided • Competitive visit differentials based on complexity and documentation requirements (Rates depend on experience, licensure, and visit type.) What You’ll Do • Provide skilled nursing visits in patient homes • Perform patient assessments and monitor clinical changes • Complete OASIS assessments when assigned • Perform skilled treatments including: • Wound care • Medication management • Injections • Catheter and ostomy care • Disease management and patient education • Provide skilled nursing visits in patients’ homes • Perform assessments and monitor patient conditions • Complete OASIS documentation for applicable visits • Deliver treatments including wound care, medication management, and disease management • Educate patients and caregivers • Communicate with physicians and clinical leadership • Complete timely, accurate EMR documentation Follow HIPAA, infection control, and safety guidelines Requirements • Active Texas RN License • 1+ year home health experience (2+ years preferred) • Strong understanding of OASIS documentation • Current CPR/BLS certification • Valid driver’s license and reliable transportation • Strong clinical assessment and documentation skills • Ability to work independently in the field Preferred Qualifications • Strong wound care experience • Home health EMR experience • Excellent patient education and communication skills Why Nurses Choose Viva Life Flexible PRN scheduling Competitive per-visit rates Supportive leadership team Locally owned and mission-driven Opportunity to grow with a rapidly expanding agency Apply today and help us deliver compassionate care where patients feel most comfortable—at home.
