Note: The job is a remote job and is open to candidates in USA. Upward Health is an in-home, multidisciplinary medical group providing 24/7 whole-person care. They are seeking a Care Management Product Manager to drive the strategy, design, and delivery of technology solutions that support clinical care operations, primarily focusing on Salesforce Health Cloud.
Responsibilities
- Drive the strategy, design, and delivery of technology solutions that support clinical care operations across the Upward Health organization
- Focus primarily on solutions built in Salesforce Health Cloud, with occasional collaboration on integrations or workflows involving other clinical applications
- Partner closely with clinical leaders, care teams, and operational stakeholders to deeply understand pain points, translate those into operational needs into well-defined product requirements, and guide solutions from concept through delivery
- Work closely with the Product Development team—including Product Analysts and Salesforce Developers—to prioritize work, define acceptance criteria, manage scope, and ensure high-quality releases
- Own the product roadmap for care management system capabilities, balancing new requests against strategic priorities while maintaining visibility into delivery progress and outcomes
- Engage with stakeholders, navigate ambiguity, and establish structure and product discipline in a fast-evolving healthcare environment
- Develop a deep understanding of clinical workflows and care team pain points to inform product strategy
- Serve as the primary product owner for Care Management capabilities, primarily within Salesforce Health Cloud
- Partner with clinical stakeholders to define requirements, document workflows, and develop clear acceptance criteria
- Translate stakeholder needs into actionable user stories and prioritized backlog items for the development team
- Collaborate closely with Product Analysts and Salesforce Developers to design and deliver scalable solutions
- Manage stakeholder expectations and proactively communicate project status, timelines, risks, and tradeoffs
- Establish clear processes for intake, prioritization, and scope management to reduce ambiguity and scope creep
- Track key metrics and outcomes to evaluate the effectiveness of delivered features and platform improvements
- Prepare presentations and status updates for leadership and operational stakeholders
- Coordinate testing, validation, and stakeholder approval for new features and enhancements
- Ensure delivered solutions align with both operational needs and broader organizational strategy
Skills
- Experience managing healthcare or clinical technology products
- Strong familiarity with care management workflows and clinical operations
- Experience working with Salesforce Health Cloud or comparable care management platforms
- Ability to translate complex operational needs into clear product requirements and user stories
- Experience writing detailed acceptance criteria and working closely with engineering teams
- Strong stakeholder management and cross-functional communication skills
- Experience managing product roadmaps and prioritization frameworks
- Ability to track product performance using metrics and outcomes
