In this role, you will serve as the primary liaison between healthcare providers and home health teams, ensuring a seamless transition for patients into home-based care.
Responsibilities
- Develop and strengthen relationships with physicians, hospitals and healthcare providers
- Coordinate home health referrals from initial referral through ongoing patient care
- Evaluate patient suitability for home health services
- Act as the key communication link between physicians, caregivers and clinical teams
- Coordinate follow-up appointments and patient care plans
- Ensure physician orders and documentation are completed accurately and efficiently
- Support the delivery of medical equipment and additional patient services where required
- Deliver educational presentations to healthcare professionals on available home health services
- Identify opportunities to grow referral partnerships and increase market presence
- Maintain accurate CRM records and activity reporting
Skills
- Formal clinical training from an accredited institution
- Active professional licence in your state of practice
- Excellent communication, presentation and relationship-building skills
- Strong organisational and problem-solving abilities
- A current driver's licence and access to reliable transportation for local travel
- Previous Home Health experience
- Experience using Microsoft Office and CRM systems
