Job Overview
As a Remote Patient Care Coordinator, you will be the primary point of contact for our members, providing guidance and support throughout their healthcare journey. You will collaborate with healthcare providers, pharmacies, and community organizations to ensure comprehensive service delivery, conduct health assessments, and develop personalized care plans tailored to each member's needs. Your exceptional communication skills, both verbal and written, will empower you to educate members on program benefits, assist with appointment scheduling, and maintain meticulous records within our care management platform.
Key Responsibilities
- Build and maintain trusting relationships with members by providing empathetic guidance and support throughout their healthcare journey.
- Coordinate care by collaborating with healthcare providers, pharmacies, and community organizations to ensure comprehensive service delivery.
- Conduct health assessments and develop personalized care plans tailored to each member's unique needs and circumstances.
- Identify and address Social Determinants of Health (SDOH) that may impact overall member well-being, connecting them with relevant resources and services.
- Communicate effectively with members via phone and text to educate them on program benefits, assist with appointment scheduling, and provide ongoing support.
- Maintain accurate and up-to-date records within our care management platform, ensuring adherence to best practices and regulatory requirements.
Essential Qualifications and Skills
To excel in this role, you will need:
- A minimum of 2 years of experience in care coordination, case management, community health, or a comparable field, with a proven track record of delivering exceptional member outcomes.
- Excellent communication skills, both verbal and written, coupled with strong problem-solving capabilities and a compassionate approach to care coordination.
- Proficiency in utilizing Apple computers (e.g., MacBook) and experience in electronic health records platforms, with the ability to quickly adapt to new technologies.
- Familiarity with motivational interviewing techniques to empower members effectively and support their health and well-being goals.
- Fluency in English is required, with Spanish language skills being highly preferred.
Preferred qualifications include:
- LPN or RN designation, demonstrating a strong foundation in healthcare principles and practices.
- Residency in Tennessee, as this role requires a work-from-home setup with a structured Monday to Friday schedule.
