We are seeking experienced Psychiatric Mental Health Nurse Practitioners (PMHNPs) to join a growing outpatient telehealth practice focused on delivering high-quality psychiatric care to adult and geriatric populations. This is a fully remote position supporting a New York–based clinic, and providers must be located within the Eastern or Central time zones with the ability to work Eastern Time hours.
This opportunity is designed for full-time clinicians who are looking to build a consistent and meaningful outpatient practice while working from home. Providers will manage an established patient population with ongoing care needs, allowing for continuity, clinical impact, and long-term patient relationships.

Requirements
Conduct comprehensive psychiatric evaluations and ongoing follow-up visits via telehealth
Diagnose and manage a wide range of psychiatric conditions in adult and geriatric patients
Develop individualized treatment plans that account for psychiatric and underlying medical conditions
Prescribe and manage psychotropic medications with careful monitoring and follow-up
Address complexities related to aging, comorbid medical conditions, and polypharmacy
Maintain continuity of care through consistent patient engagement
Collaborate with clinical and administrative teams to support coordinated outpatient care
Provide education to patients and families regarding diagnoses, medications, and treatment plans
Maintain accurate, timely, and compliant documentation within the EMR
Qualifications
Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
Active, unrestricted New York State Nurse Practitioner license
Ability to work full-time aligned with Eastern Time clinic hours
Experience treating adult populations with comfort managing geriatric patients
Strong understanding of how medical comorbidities impact psychiatric treatment planning
Prior outpatient or telehealth experience preferred
Ability to manage a consistent outpatient caseload independently
Strong clinical judgment and decision-making skills
Excellent communication and virtual patient engagement skills
Comfort with telehealth platforms and EMR systems
Spanish speaking preferred
