About the Role
As a Remote Registered Nurse (RN) Case Manager for Aeetna, you will play a vital role in supporting members' health and well-being. You'll utilize your clinical expertise to assess needs, develop care plans, and empower individuals to achieve optimal health outcomes. Responsibilities
- Conduct telephone and virtual assessments to understand members' medical needs.
- Develop and implement individualized care plans in collaboration with members.
- Proactively address challenges and identify opportunities to improve health outcomes.
- Coordinate services and programs to support members' overall health and well-being.
- Ensure compliance with regulatory requirements and quality standards.
Qualifications
- Required: 3+ years of experience with Virginia Medicaid waivers, 3+ years of oversight care insights, 2+ years of clinical practice experience, and current, unrestricted Virginia RN license.
- Preferred: Experience with Medicaid waivers.
