A Long Island nonprofit services agency is looking to add a new, licensed Registered Nurse (RN) or licensed Practical Nurse (LPN) to join their growing team on a full-time basis at one of their residential facilities in Suffolk County.
About the Opportunity:
• Setting: Residential
• Shift: Day
• Schedule: M-F 7am to 3pm OR M/W/F with Friday being 7am to 7pm
• Patient Population: 6 to 21 year old individuals with ID/DD
Responsibilities:
The Nurse will:
• Develop a person-centered plan of care that prescribes strategies and interventions
• Support discharge planning process with members of relevant teams
• Provide or coordinate the resident / family education
• Collect comprehensive data pertinent to the resident's health or the situation
• Perform all other duties as assigned
Qualifications:
• 6+ months of related Nursing experience
• Graduate of an accredited School of Nursing
• NYS Nurse (RN or LPN) license
• BLS certification
• Strong understanding of the needs of vulnerable populations including youth and young adults with Autism, Developmental Disabilities, Communication Impairments, and Medical Complexity
• Solid assessment, clinical and documentation skills
• Patient oriented
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized
Desired Skills:
• Bachelor's Degree in Nursing
• National Certification in area of specialty
