Job Description
We're looking for a dedicated and empathetic CareCoach to provide patient-centered care, support care plans, and coach individuals on maintaining a healthy lifestyle. As a CareCoach, you'll:
- Provide empathetic support and education to patients with chronic conditions
- Develop strong relationships with patients through regular communication and follow-ups
- Coach patients on healthy lifestyle choices, including diet, exercise, and medication adherence
- Encourage and guide patients in using vital monitoring tools to manage their health effectively
- Coordinate patient care across multiple providers and services to ensure comprehensive care
- Work efficiently with various Electronic Medical Records (EMR) systems, including EPIC, eClinicalWorks, and Practice Fusion
Requirements
- Certified Medical Assistant (CMA) certification required
- Experience in chronic care management and patient education
- Strong understanding of various chronic conditions and their management
- Proficiency in using EMR systems
- Excellent communication and interpersonal skills
- Ability to work independently in a remote setting
- Passion for patient care and curiosity about advancements in AI technology
- Bilingual proficiency in Arabic or Spanish strongly preferred
