Treatment Foster Care Case Manager Location: Boise, Idaho; Nampa, Idaho; Caldwell, Idaho; Garden City, Idaho; Meridian, Idaho; Star, Idaho; Kuna, Idaho; Payette, Idaho Position Type: Full-time Remote Status: Hybrid / Community-Based Salary: $20–$24 per hour Application Deadline: Applications will be reviewed on a rolling basis until the position is filled. About the Role Clarvida is seeking a Treatment Foster Care Case Manager to support youth with significant behavioral, emotional, or medical needs in a family-based treatment setting. This role is an essential member of the Treatment Team, coordinating behavioral health and community-based services to help youth stabilize, achieve treatment goals, maintain placement, and work toward permanency with their families. As a Case Manager, you will collaborate closely with foster parents, clinicians, the Idaho Department of Health & Welfare, and birth families to develop individualized treatment plans and ensure consistent progress. Responsibilities Develop individualized treatment goals and service plans for children and youth in Treatment Foster Care. Coordinate behavioral health, educational, and community-based services. Monitor treatment progress, update goals, and support readiness for discharge. Provide regular communication and support to foster parents. Partner with birth families as appropriate to support permanency planning. Maintain accurate documentation and timely progress notes. Participate in clinical supervision, team meetings, and ongoing training. Required Qualifications Bachelor’s degree (Human Services field preferred): Social Work, Psychology, Sociology, Counseling, Family Relations, Nursing, Occupational Therapy (OT), Physical Therapy (PT), Therapeutic Recreation (TR), Speech-Language Pathology (SLP), Behavioral Sciences, Art Therapy, or related field. Ability to pass background checks, including Enhanced Fingerprinting with Idaho DHW. Valid driver’s license, reliable transportation, and proof of auto insurance.
